H & J Counseling accepts the NMAP philosophy of service provision, which includes
acceptance of, respect for, and a positive attitude toward Medicaid and non-Medicaid
clients, and the philosophy of client empowerment. H & J Counseling ensures that
the partners and any future employees will possess appropriate licensure or certification
to provide services and will remain current with the requirements of the licensure
or certification. In addition, H & J Counseling will maintain up-to-date and accurate
provider agreements. As mentioned previously, through initial assessment H & J Counseling
will obtain adequate information on the medical and personal needs of each client.
H & J Counseling will not discriminate against any client, employee, or applicant

origin. In addition, H & J Counseling agree to make a law enforcement check and
Adult Protective Services and Child Protective Services Central Registry check a
part of any employee application process. In order to best serve our clients any
appropriate contacts will be made with client release. These contacts may include
communication with school staff, employers, the client's family, probation officers,
case managers and other mental health providers. In accordance with our mission,
H & J Counseling will encourage children and families to identify, utilize and build
upon their existing abilities through individually tailored therapeutic and other
family centered services.
H & J Counseling will comply with Title VI of the Civil Rights Act of 1964, Title
IX of the Education Amendments of 1972, Section 504 of the Rehabilitation Act of
1973, the Age Discrimination in Employment Act of 1975, the Americans with Disabilities
Act of 1990, to the effect that no person shall, on the grounds of race, color,
national origin, sex age, handicap or disability, be excluded from participation
in, denied benefits, of or otherwise be subjected to discrimination under any program
or activity for which H & J Counseling is providing a service for.

H & J Counseling ensure that it will operate a drug-free workplace in accordance
with Nebraska State guidelines. We agree that the unlawful manufacture, distribution,
dispensation, possession, use, or being under the influence of a controlled substance
or alcohol in the work place or during the performance of work is strictly prohibited.
